Welcome to “Breathe: Healthy Steps to Living Tobacco Free!” Breathe is a program funded by the Indiana Tobacco Prevention and Cessation Commission (TPC). Partner organizations who have completed a Breathe training may access and utilize the following digital materials. (If your organization has not completed a Breathe training or would like hard copies of these materials, please contact Tanya Shelburne at tanyas@healthedpros.org.) Breathe Flyer master and Breathe Promotional video provide additional information about Breathe and the Breathe kits.
If your staff has completed a Breathe training, feel free to print and post these flyers to give your staff easy access to the Breathe digital materials!
Flip Chart
The Breathe Flip Chart (updated) is for trained staff to use when talking to parents, either individually or in small groups. One side shows basic information for the parent and the other side includes talking points and resources to guide you in discussing each topic. You can utilize a single page for a topic that needs to be addressed or walk parents through several pages of the flip chart. On pages with QR codes you can use a smartphone or tablet in camera mode to access the short video on that topic.
Children’s Activities Booklet
The Breathe Children’s Activities Booklet is also for trained staff to use in classrooms (geared towards ages 3-7) to teach basic lessons on healthy bodies. These activities are short, require very little prep time or materials, and are not overtly focused on tobacco.
Parent Activities Booklet
The Breathe Parent Activities Booklet is for trained staff to use with groups of parents/guardians to engage them in short activities/discussions around specific tobacco-related issues. These activities are great for parent nights or other group meetings and require minimal prep time or materials.
Breathe Videos
On this Vimeo page, you will find numerous short Breathe videos for parents on various tobacco-related issues as well as demonstrations of several of the children’s activities. These videos can be shown to parents during education sessions, home visits, parent/teacher conferences, etc., and may also be posted on your social media pages.
Parent Handouts
The Breathe Parent Handouts (pdf of all 22 handouts (updated) can be printed or shared digitally with parents as part of your tobacco education programming or as specific needs arise. The handouts can also be posted on your social media pages.

Organizations that have a text message system for their parents can pick and choose from the following 100 text messages to send out as desired. Or automate your system to send all 100 messages over the course of the year to keep tobacco education on your parents’ radar and offer resources for those ready to quit.

You can order a variety of FREE Quit Now Indiana materials here. Place these items in public areas and have them on hand as you do tobacco education so you can share this evidence-based resource with those who are ready to quit tobacco.
Parent/Guardian Surveys
Parent/Guardian surveys help us collect meaningful data, improve the Breathe program, and give you another opportunity to bring up the topic of tobacco! As an incentive, any parent/guardian who completes a survey (and provides their contact information) will be entered into a quarterly drawing for a $50 Walmart gift card!
There are multiple ways these surveys can be administered and they can be completed at any time during the year (before or after providing tobacco education). If someone requests help with quitting (and provides contact information), we will follow up to offer support and resources.
Parent/Guardian surveys could be completed at enrollment nights, during parent-teacher conferences, at home visits, in one-on-one or group settings, etc.
Any of the following formats can be used:
- Online in English OR Online in Spanish
- Hard copies of the Survey in English OR Hard copies of the Survey in Spanish (return completed surveys to your Breathe trainer)
- Distribute cards with QR codes that link to the online surveys (supplied by your Breathe trainer)
- Print out this Parent/Guardian Survey QR Code Flyer and post it in your organization so parents/guardians can scan the QR code with their smartphone to take the survey.
- Include the following sample text in your organization’s newsletter or in an email to the families you serve: “Parents/Guardians…please take a few minutes to fill out a short survey from our tobacco education partner! Any parent/guardian who completes a survey (and provides their contact information) will be entered into a quarterly drawing for a $50 Walmart gift card!
