Timeline

Community-based Funded Partners that selected the Tobacco Free Families indicator should follow the suggested timeline below* in order to meet the required deliverables:

August

Year 1

  • Attend the Advanced OR Beginner Breathe Training for Funded Partners webinar in August; anyone who missed the webinars or new coordinators hired after August will need to watch the recording and follow the steps outlined here.
  • Thoroughly review the Breathe website/online instructional guide
  • Sign up for the monthly Breathe e-newsletter
  • Join the private Breathe Facebook group, optional

September-December

Year 1

  • Make contact with the leadership of local Head Start and other potential partner organizations
  • Hold introductory meetings with potential new partners or refresher meetings with existing Breathe partners
  • Complete/Update Key Informant Interview, optional
  • Initiate a Memorandum of Understanding (MOU), optional
  • Set date for the Initial Breathe training for the organization’s staff; Notify HEP of training and request Breathe materials at least TWO weeks before the training

September-March

Year 1

  • Conduct Breathe training for the organization’s staff
  • Complete all post training steps
  • Review evaluation reports from Health Ed Pros and implement program updates and recommendations as appropriate

October-June

Year 1

  • Check-in with the organization’s leadership at least once a month (see tips for maintaining a relationship)
  • Review the agency’s tobacco policy and provide feedback, optional
  • Work with Breathe partners to add questions to their health assessment forms for the next school year, optional; request a summary of any useful data captured this school year
  • Conduct Community Education, as appropriate

April-June

Year 1

  • Present the organization’s leadership with a Breathe Certificate of Appreciation
  • Issue a press release acknowledging your partnership, optional
  • Meet with the organization’s leadership to determine what worked well and what didn’t this year and begin discussing plans for next year, including possible summer dates for a Breathe Refresher for returning staff or another Initial Breathe training for new staff or another segment of their staff that was not previously trained

July-August

Year 2

  • Attend the Annual Breathe Training for funded partners (details to come); anyone who misses the training as well as new coordinators hired in grant year 2 will need to watch the appropriate recordings 
  • Identify additional potential partners and begin reaching out

September-June

Year 2

  •  Repeat the same steps in the timeline for the previous year with at least 1 new partner, see above
  • Conduct a Breathe Refresher for the organization trained in year one or another Initial Breathe training for new staff or another segment of their staff that was not previously trained; adhere to the same schedule as the previous year for requesting materials and submitting evaluation components

Ongoing

  • Share the monthly Breathe newsletter with your Breathe partners
  • Provide and replenish Quit Now Indiana materials for each of your partners
  • Work with Breathe partners to administer parent surveys
  • Communicate regularly with Breathe partners to assess the impact of Breathe and address questions and concerns that arise
  • Engage in the Breathe Facebook group to get additional support and offer your ideas to other funded partners, optional

* For established Breathe partners this timeline may differ depending on the date of their previous training. Ideally, all Breathe partners will receive training every year.
