Protocol for Pre and Post Assessments
- *Each Breathe training should begin with participants completing the Breathe Pre-Training Assessment Updated 7.7.23 either in hard copy or online at this link.
- *Each Breathe training session should end with participants completing the Breathe Post-Training Assessment Updated 7.7.23 either in hard copy or online at this link. Be sure to allow plenty of time so these forms can be completed by all training participants.
- *WITHIN ONE WEEK of the training, IF ANY HARD COPY ASSESSMENTS WERE USED input the pre assessments into this survey link: PRE-ASSESSMENT. Input all post assessments into this survey link: POST-ASSESSMENT.
- Health Ed Pros will generate reports of the pre and post-assessment data and share these reports with you and TPC.
- *Review the report and use this information to inform future trainings as well as ongoing work with that particular partner. At your discretion, you may choose to share the report with the organization’s leadership.
- At the end of each grant year, Health Ed Pros will generate a summary report of all pre and post-assessment data collected across the state. This report will be shared with TPC and funded partners and will be used to determine what is working well and what changes need to be made.
