Parent Surveys

The New & IMPROVED Parent/Caregiver Survey is now available! Spanish survey coming soon.

Purpose

  • Parent/Caregiver surveys provide a tangible opportunity to engage with parents at events and through other communication channels, and the surveys put tobacco education on their radar
  • The surveys allow us to gather data about tobacco use from the population we aim serve with Breathe, and they provide an opportunity to share personal tobacco stories, which gives us qualitative insights
  • The surveys give us feedback on how parents feel about tobacco education, which we can use to modify Breathe

Goal

Our goal is to collect 250 surveys by the end of May, 2027…and we need your help! Make sure your county is represented. Any county that collects at least 10 surveys will be given an individualized report in June 2027.

Survey Implementation

There are multiple ways the surveys can be administered:

  • Funded Partners AND Breathe Partners can collect parent surveys. Talk to each of the organizations you have trained and work with them to develop a survey implementation plan that works best for them. You can use and personalize the Sample Text in this document to communicate with your Breathe Partners about the surveys.
  • These surveys could be completed at enrollment night, during parent-teacher conferences, at-home visits, in one-on-one or group settings, or by sharing on social media, in newsletters or with posted flyers.
  • The surveys can be completed at any time during the year (before or after providing tobacco education).
  • The survey includes built-in skip logic. Therefore, these surveys can only be filled out online (no paper/pencil versions are available).
  • If someone requests help with quitting on their survey (and provides contact information), Health Ed Pros will share the information with the appropriate County Coordinator, who should then follow up to offer support and resources, including Quit Now Indiana information.

Incentives
